A. About UP High School for the Arts
Envisioned to develop creative nationalist thinkers and leaders in a dynamic global society, The UP High School aims to prepare students in the field of arts in harmony with information technology for a two-pronged direction: tertiary education or a career or gainful employment in the creative industries.
B. Schedule of Admission Activities
March 1, 2012 – Last day for filing of HSAT application
March 2, 2012 – Giving out of test permits
March 3, 2012 – HSAT (High School Admission Test)
March 10, 2012 – Issuance of notice of qualified applicants for the talent test
March 14, 2012 – Talent Test
March 19, 2012 – Giving out of final and official results to each examinee
April 2, 2012 – Pre-enrollment of qualifiers
April 3, 2012 – Pre-enrollment of waitlisted qualifiers
C. Qualifications for Admission
To qualify for the HSAT, an applicant must:
- be an elementary school graduate by March 2012;
- not have previously studied in another high school;
- have an average grade of 85% or better in Math, Science, English/Reading, Civics/Social Studies, and Filipino subjects at the time of filing of application; and
- have a conduct grade of B or its equivalent.
D. Supporting Documents Required
- Two (2) copies of RECENT 2×2 colored or black & white photo with the applicant’s full name written at the back;
- A certified true copy of grades (signed by the Principal or the Adviser) of the latest grading period (NOTE: A photocopy without the original signature of the Principal or the Adviser will not be honored.);
- A copy of the BIR Income Tax Returns or W-2 of the last two years (2009 and 2010) of both parents or guardians. (NOTE: Applicants from public schools whose parents have no fixed income must secure from the UP High School office a form to be filled out. This document, which would show their financial status, must be certified or attested to either by the Principal or the Guidance Counselor of their school.);
- A copy of the business tax of the last two years (2009 and 2010) for those engaged in business;
- A copy of the latest electric bill;
- A signed waiver of admission of parents whose joint annual gross income exceeds PhP150,000.00; and
- A clear vicinity sketch or map of the applicant’s residence.
E. Steps in the Application Process
- The applicant, together with his/her parent, submits the complete documents, fastened in a long folder, to the receiving clerks (Analie or Alice).
- The applicant fills out the HSAT Application Form.
- A faculty member then a) checks if the application form is properly filled out, b) interviews the applicant, and c) interviews the accompanying parent/guardian.
- The accompanying parent signs the sworn statement on the HSAT Application Form.
IMPORTANT: UP High School Cebu may refuse acceptance of incomplete documents or improperly filled out forms. Also, it reserves the right to perform a social investigation (SI), if needed, to verify the accuracy and veracity of the information submitted by the applicant.
F. Fees
An application fee of one hundred pesos (P100.00) must be paid by the applicant.
G. Coverage of the High School Admission Test (HSAT)
A. The HSAT is administered using the English language (except for the section on Filipino) and covers the following subjects:
- Mental Ability
- English
- Filipino
- Mathematics
- Social Studies
- Science
B. Talent Test
The applicants will be ranked based on their HSAT written test scores. The top eighty (80) applicants will show their talents before a panel of three members. The talent may be in any of the following: drawing, painting, singing, dancing, acting, elocution (oration and declamation). Applicants are encouraged to bring with them a portfolio of their work/certificiates/awards received.
Applicants must bring their own accompanying music or art supplies for the talent test.
H. HSAT Passers
The top forty (40) applicants based on the HSAT written test and talent test qualify for admission to the UP High School Cebu. After they have been informed of passing the HSAT, they must:
- Confirm their intent to enroll by registering and submitting the original Report Card and photocopy of birth certificate for pre-enrollment on April 2, 2012. If the original Report Card is unavailable for submission, a certification from the Principal on the exact date of its release will suffice. Failure to confirm by 4:00p.m. on the said date will mean forfeiture of the slot.
- Pass the Physical Health Examination (PHE) to be conducted by the school physician of UP Cebu. Date of the PHE will be announced during the pre-enrollment.
- Have their application forms notarized in preparation for enrollment.
- Follow the official enrollment process during the regular enrollment period. Enrollment date will be announced later.
I. Reminders for Waitlisted Qualifiers
Waitlisted qualifiers should confirm their intent to enroll by submitting to the High School office their waitlisted notice not later than 9:00 a.m. of April 3, 2012. Announcement of available slots will be done at 9:00a.m. on the same day. Failure to confirm their interest to enroll forfeits their right to a slot.
A waitlisted qualifier who will be admitted should submit the original Report Card and a photocopy of his/her birth certificate. If the original Report Card will not be available for submission, a certification from the School Principal on the exact date of its release should be submitted.

Subject: Waiver of Admission as Per Specified Condition
Sir/Madame:
We hereby declare that we have full knowledge of the UP High School Cebu democratized admission policy which limits admission only to students who belong to families with annual gross family income of PhP150,000.00 or lower.
We also declare that we have full knowledge of the proposal to open admission to applicants who belong to families with annual gross income higher than PhP150,000.00.
We are fully aware that the proposed open admission becomes effective and applicable ONLY upon the final approval of the Board of Regents.
Thus, in the event that the said proposal does not get the final approval for implementation for SY 2011-2012, we, parents of __________________________________ hereby willfully and voluntarily execute this waiver of admission. Our child ___________________________ does not qualify for admission regardless of the results of the High School Admission Test (HSAT) because our gross family income exceeds the required income ceiling under the current democratized admission policy.
